Benefits
If you are a full-time employee, you will enjoy all of the benefits described
within as soon as you meet the eligibility requirements for each particular
benefit. If you are a part-time employee, you will enjoy only those benefits
that are required by law to be afforded to you, provided that you meet
the minimum requirements set forth by law and in the benefit plan(s).

Funeral (Bereavement) Leave
You are entitled to take up to two (2) workdays with pay to attend the
funeral and take care of personal matters related to the death of a member
of your immediate family.
Personal Leave
As a full-time employee you are eligible to take two (2) days of paid
personal leave during each calendar year.
Insurance Coverage
Group Insurance
Pharma Logisticsâ is interested in the health and well being of both you
and your family. A comprehensive health insurance program is available
for you and your family. We provide group insurance underwritten by a
national insurance carrier. When you choose insurance coverage, our insurance
company provides a booklet describing your benefits; a copy of this will
be given to you when you join the program.
Prescription Drug Plan
The plan pays the participating pharmacy rate for prescription drugs after
a co-payment ($9 for 2001) for each 34-day prescription or refill. The
plan covers up to a three-month supply of a maintenance prescription drug
after you pay a co-payment for each month's supply.
Workers' Compensation
The Illinois Workers' Compensation Law is a no-fault insurance plan that
is supervised by the state and one hundred percent (100%) paid for by
Pharma Logisticsâ. This law was designed to provide you with benefits for
any injury that you might suffer in connection with your employment. Under
the provisions of the law, if you are injured while at work, you are eligible
to apply for Workers' Compensation.
Unemployment Compensation
Pharma Logisticsâ pays a percentage of its payroll to the Unemployment
Compensation Fund according to Pharma Logistics'â employment history. If
you become unemployed, you may be eligible for unemployment compensation,
under certain conditions, for a limited period of time.
Social Security
The United States Government operates a system of contributory insurance
known as Social Security. As a wage earner, you are required by law to
contribute a set amount of your weekly wages to the trust fund from which
benefits are paid. As your employer, Pharma Logisticsâ is required to deduct
this amount from each paycheck you receive. In addition, Pharma Logisticsâ
matches your contribution dollar for dollar, thereby paying one-half of
the cost of your Social Security benefits.
